Re-treatment of an existing root canal

Re-treating a previously root-treated tooth is more complex and time-consuming than a primary root canal. A specific written quote is provided after radiographic assessment.

The root canal fee covers the endodontic treatment itself. The tooth must then be definitively restored (filling or crown) to seal it against bacterial leakage and to protect it from fracture — this is quoted separately in writing. What's Included in the Root Canal Fee

Every root canal at Smile Dentist London is carried out by a GDC-registered dentist using modern endodontic protocols. The fee covers all of the following — the pre-treatment consultation is charged separately.

Local anaesthetic

Effective numbing of the tooth and surrounding area so the procedure is comfortable.

Rubber-dam isolation

A soft sheet that keeps the tooth dry and clean during treatment, in line with British Endodontic Society guidance.

Access and canal location

A small access cavity is made through the top of the tooth and all root canals are located and explored.

Cleaning, shaping & irrigation

Modern nickel-titanium rotary files are used together with antimicrobial irrigants to clean and shape the canals to a precise apex-locator-verified length.

3D sealing (obturation)

The canals are sealed three-dimensionally with gutta-percha and a biocompatible sealer to prevent re-infection.

Core filling & written aftercare

A temporary or permanent core filling is placed at the end of the appointment, and written aftercare instructions are provided.

What to expect

Your Root Canal Treatment Journey

Many root canals can be completed in a single appointment where clinically appropriate. The steps below describe a typical journey; your dentist will give you a personalised plan after assessment.

Visit 1

1.Consultation, diagnosis & written treatment plan

Clinical and radiographic assessment to confirm the diagnosis (irreversible pulpitis or apical infection) and to assess whether the tooth is restorable. Realistic alternatives — root canal, extraction with replacement (implant, bridge, denture), or monitoring — are discussed. You receive a written, itemised treatment plan before any treatment is started.

2.The root canal appointment

Under local anaesthetic and rubber-dam isolation: access to the pulp chamber, location of all canals, cleaning and shaping with nickel-titanium rotary instruments, antimicrobial irrigation, electronic apex-locator measurement of canal length, three-dimensional sealing of the canals with gutta-percha and a biocompatible sealer, and a core filling to seal the tooth.

Days–weeks after

3.Settling period

Mild tenderness when biting is normal for a few days and usually settles with ordinary painkillers. We provide written aftercare and a phone number to contact us if symptoms are not resolving.

Definitive restoration

4.Crown or final filling

A permanent restoration is placed to protect the tooth from fracture and to seal it against bacterial leakage. For back teeth (premolars and molars) this is usually a crown; for front teeth a high-quality filling is often sufficient. The most appropriate restoration for your tooth is discussed and quoted separately in writing.

Root canal is often recommended for

  • Irreversible pulpitis (severe, lingering toothache, often worse with hot/cold)
  • A dental abscess or infection at the root of the tooth (apical periodontitis)
  • A tooth that has died (become non-vital) after trauma or deep decay
  • Preserving a tooth that is otherwise restorable, as an alternative to extraction
  • Re-treatment of a previously root-treated tooth where infection has recurred

Root canal may not be the best option when

  • The tooth is fractured below the gum line or is otherwise unrestorable
  • There is advanced gum disease affecting the tooth that cannot be predictably treated
  • Very little tooth structure remains to support a final restoration
  • You prefer to replace the tooth with a dental implant after extraction (this is a separate discussion)
  • The tooth is non-functional and not worth preserving — extraction may be more appropriate

The right choice depends on a clinical and radiographic assessment by your dentist, who will explain the realistic options and prognosis for your individual tooth before any treatment is agreed. No dental treatment is guaranteed.
